Blood Glucose
Diabetes Partner interfaces with over 24 different blood glucose meters to accurately record and store an unlimited number of daily readings; the software also allows diabetes patients to manually enter test results and identify factors (such as exercise, stress or illness) that may have moved a particular reading out of the target range.
Health Analysis Charts and Graphs
Diabetes Partner uses a variety of color-coded charts and graphs to identify when blood glucose levels are above, below or on target, and track medication and diet. Patients can attach comments about factors, such as stress or exercise, that may have affected a particular reading, and print the Health Analysis charts and graphs for evaluation by their care providers.

Diabetes Partner includes an electronic log book for recording the amount of insulin and/or oral agents administered during established timeslots each day.
Food Intake
Diabetes Partner also incorporates a patient's diet plan with a log book to record and
store daily nutritional intake; a USDA database with the nutritional breakdown of
more than 7,400 food items; and a repository for personal recipes and favorite meals,
as well as a corresponding breakdown of carbohydrate, protein, fat and caloric counts.
Diabetes Self-Management Reminders
As a complete diabetes management tool,
Diabetes Partner
incorporates the personal care protocol established by the care team and reminds
patients when vital factors (such as the blood glucose level) fall out of the recommended
range. The software also reminds patients when they fail to record medication, meals
or blood glucose readings.
